All School of Dance Faculty and Staff absences must be requested and reported using the Absence Request & Reporting form below. Planned absences must be requested in advance and must have prior approval from your direct supervisor and the Administrative Manager. When possible, please complete the form at least 2 weeks prior. In cases of unforeseen absences, absence reporting must be completed within 3 days of the absence.

Emergency Response Coordination for SOD classes held during normal business hours is handled by the Administrative Manager. Evening and weekend emergency response during SOD productions is handled by the Production Director. If a student, faculty, staff, or community member is in imminent and immediate danger, please call:

Police: 911For Incidents / Accidents that happen either in the School of Dance, in another building related to School of Dance classes, or elsewhere in regards to School of Dance classes, productions, or faculty research, please fill out the Incident / Accident Form below and forward the confirmation email to the School of Dance Administrative Manager and Administrative Assistant. This form must be completed by the faculty / staff member that witnessed the incident--not the injured party. For questions regarding this form, please reach out to the School of Dance Administrative Manager.
Below is the report that you should fill out if you see something dangerous on campus (e.g., trip hazard, unmaintained equipment) or if you have a near miss with an injury (e.g., something fell from the ceiling near you, a piece of safety equipment failed while you were using it).

The documents below will need to be sent to the Guest Lecturer / Artist before they come to class. The guest will need to complete all paperwork and submit to the Administrative Manager before attending the class. Please check with the Administrative Manager before reaching out to guests to make sure funding is available for your class.

Teaching Independent Study courses is not required of faculty members. Offering Independent Study courses can create a significant amount of additional work for faculty, but faculty do not receive teaching load for these types of courses. If undergraduate students request Independent Study courses, faculty should encourage students to connect with an academic advisor and explore other options to fulfill degree requirements.
SOD faculty must receive Director approval to offer independent Study courses. Director approval must be received prior to a faculty member committing to a student and prior to student registration for the course. Director review is important to 1) ensure the Director can ascertain a broad view of curricular needs, ensuring that Independent Study offerings do not overlap with the existing curriculum, and 2) to ensure the Director can accurately assess faculty workload.

This memo is sent regarding the annual posting required by OSHA. The OSHA Form 300A Summary must be posted throughout the University in order for the University to comply with Occupational Safety and Health regulations.
A recorded injury or illness under the OSHA system does not necessarily imply that management or the worker was at fault, a violation of an OSHA standard has occurred, or that the injury or illness is compensable under worker's compensation.

Students and instructors in the School of Dance are expected to follow the SOD Grading Policy.
Students registered for courses in the School of Dance may appeal a final grade in accordance with University Policy 6-100. Note that students must demonstrate that the grade was either a mistake or “arbitrary and capricious.” The appeals process can be found as part of the SOD Grading Policy.
